Section 1What this notice covers
ProLife Home Care does not exclude people or treat them less favorably because of any of the characteristics named above. This applies to everyone we work with: the people we care for, their families, the aides and nurses we employ, and anyone who contacts us to ask about care.
Section 2Free aids and services
ProLife Home Care provides, at no cost to you:
- Help communicating if you have a disability. Qualified sign language interpreters, and written information in other formats, including large print, audio and accessible electronic formats.
- Help if your primary language is not English. Qualified interpreters, and information written in other languages.

Section 3How to file a grievance with us
If you believe that ProLife Home Care has failed to provide these services, or has discriminated in another way on any of the grounds named above, you can file a grievance with our Section 1557 Coordinator:

You can file a grievance in person, by mail, by fax, or by email. If you need help filing a grievance, our Section 1557 Coordinator will help you. Filing a grievance will never affect the care you or your family receive.
Section 4Filing a complaint with the federal government
You can also file a civil rights complaint with the U.S. Department of Health and Human Services, Office for Civil Rights. You do not need our permission, and you do not need to tell us that you have.
