Does Medicaid cover home care in New York, and does my mom qualify?

My mom is 81 and lives alone in Queens. She needs help with bathing and getting dressed, and she's not steady on her feet anymore. Does Medicaid pay for home care in New York, and how do we know if she would qualify?
