Before you start
What documents do you need
Bring everything at once and you can start working sooner. We cannot process an application until the list is complete.
Employment documents
- Two forms of identification, one with a photo
- Social Security card, the original
- HHA or PCA certificate, the original, if you have one
- Two reference letters
- Application form, filled out in our office
Medical requirements
- Physical exam form, signed, stamped and filled out by a doctor
- Evidence of PPD, updated every year
- Chest X-ray instead of PPD, if your PPD was positive
- Rubella titer or rubella vaccine, as a separate report
- Measles (rubeola) titer or vaccine, if you were born on or after January 1, 1957
- Drug screen, as a separate report, updated every year
