Forum Terms

Last updated October 15, 2026. These terms apply only to the Caregiver Forum at prolifehc.com/questions/. They supplement our general Terms of Use; if they conflict on a forum-specific issue, these Forum Terms control. They do not change any patient care agreement or legally required patient rights.

1. Eligibility and acceptance

The Forum is intended for adults aged 18 or older. You must use an email address you control and may not create multiple accounts to evade moderation. By selecting the unchecked acceptance box and creating an account, you agree to these Forum Terms and the incorporated Forum Rules. We provide the Privacy Policy separately for your review. We record the version and time of acceptance. If you do not agree, do not register or post.

2. Public forum; no professional relationship

The Forum provides general information and peer discussion, not individualized medical, nursing, legal, financial or Medicaid eligibility advice. Reading or receiving a response does not itself establish a patient-provider relationship. Do not change medication or treatment based on a post. This is not an emergency or continuously monitored service. Call 911 for immediate danger; call or text 988 for a mental-health crisis. Contact your own licensed clinician for personal medical decisions.

3. Privacy and patient information

Posts are public and may be copied, indexed, archived or quoted by others. Do not post patient records, identifying information about another person, or confidential information obtained through caregiving or employment, even if you believe the person would not object. Do not assume a screen name or anonymous-post label makes the content unidentifiable. Do not submit requests about an existing ProLife patient’s care through the public Forum; use the agency’s designated private channels. Our Privacy Policy explains collection, moderation, retention and deletion.

4. Account security

Protect your credentials and notify Team@ProLifeHC.com of suspected unauthorized access. We may require account verification, restrict abusive use or disable compromised accounts. Google sign-in, when offered, is subject to the applicable Google terms.

5. Your content and license

You retain ownership of your original content. You grant ProLife a nonexclusive, worldwide, royalty-free license to store, reproduce, display, format, translate and distribute your content solely to operate, moderate, secure and promote the Forum and its discussions, including forum notification emails and search indexing. This license lasts while the content remains available on the Forum and for a reasonable period needed for backups, legal obligations and previously distributed copies. When we remove content from our active systems, we cease new authorized uses, except where legally necessary. This license does not authorize using your health story in paid advertising without separate permission. You represent that you have rights to submit your content and that it does not unlawfully disclose another person’s information.

6. Moderation and reports

The Forum Rules are incorporated into these Terms. We may review, delay, remove, or restrict content or accounts to enforce rules, protect users and comply with law. Automated tools may flag content; human review may be requested using the notification process described in the Privacy Policy. Moderation is not a guarantee that unlawful, inaccurate or unsafe posts will be detected. We may remove identifying details without changing the substantive meaning; otherwise we may remove the post. Reports involving threats, suspected patient abuse or agency complaints may be escalated outside ordinary forum moderation according to applicable law and internal policy.

7. ProLife employees and professional boundaries

Employees posting in an official capacity will be clearly identified as “ProLife Home Care employee” or “ProLife team” with role and professional credentials where applicable. They must not publicly confirm anyone’s status as a ProLife patient, disclose information from care records, or provide individualized treatment orders through the Forum. Mentions of ProLife services must clearly disclose the employment relationship. Official answers remain general information and do not replace the agency’s formal patient, complaint or clinical channels.

8. Copyright complaints

Send copyright complaints to our designated DMCA agent below. A notice should identify the work, the allegedly infringing material and its URL, your contact details, a signature, and the good-faith and accuracy/authority statements required by 17 U.S.C. §512(c)(3). Counter-notices may be submitted under §512(g). We may terminate repeat infringers in appropriate circumstances.

9. Disclaimers and limits

The Forum is provided “as is” and “as available” to the extent allowed by applicable law. We do not guarantee availability, accuracy or completeness of user submissions. To the extent permitted by law, ProLife is not responsible for indirect or consequential damages caused solely by use of this free Forum. Nothing in these Terms excludes or restricts liability that cannot lawfully be excluded, patient rights, statutory duties, or obligations arising from licensed home care services.

10. User responsibility

To the extent allowed by law, you are responsible for claims arising from your unlawful content or material breach of these Terms. Any indemnification obligation is limited to losses reasonably attributable to your conduct and does not apply to ProLife’s own negligence, unlawful acts or independent obligations.

11. Governing law and disputes

New York law governs these Terms, except where mandatory law provides otherwise. Subject to applicable jurisdiction and venue requirements, disputes relating solely to the Forum may be brought in courts of competent jurisdiction in Kings County, New York. Nothing here restricts legally protected access to agencies, regulators or small-claims court.

12. Changes, suspension and deletion

We may modify these Terms prospectively and will provide reasonable notice of material changes, including email or a prominent in-product notice. Where law or the nature of a change requires renewed consent, we will request it. You may stop using the Forum and request account and content deletion as described in the Privacy Policy. We may suspend or discontinue the Forum, subject to applicable legal obligations.
